Consent to Treatment
By attending an appointment at Flex & Align, you acknowledge that you willingly give consent to our physiotherapists and allied health practitioners to perform any examinations, assessments and treatment procedures considered appropriate in accordance with their professional training, qualifications and clinical judgment.
You understand that you have the opportunity to discuss the nature and purpose of your treatment with your practitioner before any treatment is performed, and that you may refuse or withdraw consent to treatment at any time.
You acknowledge and accept the following:
1. Treatment
1.1 Physiotherapy and allied health care may involve physical treatment, including hands-on techniques, exercise prescription, rehabilitation, education, and other treatment options.
1.2 You have the right to choose the practitioner you wish to see, refuse treatment, or seek a second opinion at any time.
1.3 There are risks associated with treatment, including temporary stiffness, soreness, soft tissue irritation, neurological complications, skin reactions, bruising and other minor complications.
1.4 We conduct thorough assessments and take reasonable precautions to minimise the risk of adverse outcomes.
1.5 In many cases, alternative treatment options exist. If your condition falls outside the scope of physiotherapy or allied health care, or if treatment is considered inappropriate, your practitioner may refer you to another healthcare provider.
1.6 Choosing not to undertake treatment may result in ongoing pain, reduced mobility, reduced function, delayed recovery, or progression of your condition.
2. Consent to Obtain and Release Information
2.1 To assist in the management of your condition, rehabilitation or claim, it may be necessary for Flex & Align to communicate with other treating practitioners, specialists, rehabilitation consultants, insurers, employers, case managers or legal representatives involved in your care.
2.2 By accepting these Terms and Conditions, you authorise Flex & Align to obtain and exchange relevant medical and health information where reasonably necessary for your treatment, rehabilitation or claim management.
3. Exclusion of Liability
3.1 Flex & Align and its directors, employees and contractors will not be liable for any injury, loss or damage arising from treatment provided by the clinic except where such liability arises from negligence or where liability cannot be excluded under Australian law.
4. Privacy and Contact
4.1 Flex & Align may send appointment confirmations and reminders by SMS, email or telephone.
4.2 From time to time, you may receive updates, surveys, promotional offers or information about services provided by Flex & Align.
4.3 You may opt out of marketing communications at any time.
4.4 Your personal information will not be disclosed to third parties except where required for your treatment, rehabilitation, claim management, or where required or authorised by law.
4.5 Our handling of personal information is governed by our Privacy Policy.
5. Payments and Billing
5.1 Payment for consultations and products is required on the day of your appointment unless otherwise agreed by Flex & Align.
5.2 Payment may be made in person, over the phone, or by another payment method approved by the clinic.
5.3 Refunds for products are provided in accordance with the Australian Consumer Law. Change-of-mind refunds are not available for personal or healthcare items.
5.4 Our billing policies are designed to support the ongoing provision of high-quality healthcare services.
6. Late Arrivals and Missed Appointments
6.1 If you arrive late but within your appointment time, treatment may still be provided for the remaining appointment duration.
6.2 If you fail to attend your appointment or arrive after your allocated appointment time has ended, a cancellation fee of $50 may apply.
6.3 If an appointment can be rescheduled within the same business day, the cancellation fee may be waived at the discretion of Flex & Align and subject to practitioner availability.
6.4 Appointments may be cancelled or rescheduled without charge by providing at least 6 hours' notice prior to the scheduled appointment time.
7. Satisfaction Guarantee
At Flex & Align, we are committed to providing exceptional care and service. If you are not 100% satisfied with the service you received, we will provide your next consultation free of charge, subject to the following conditions:
7.1 A claim must be made before your next consultation or within 7 days of the original appointment.
7.2 The guarantee applies to one consultation only and not to treatment packages or multiple-session programs.
7.3 The complimentary consultation must be of the same or lesser value than the original consultation.
7.4 The complimentary consultation must be used within 7 days of the original appointment.
7.5 The guarantee is not redeemable for cash, products, refunds or transfers.
